1. Eligibility: The Dunk Health Contest (the "Contest") is open to all individuals who are at least 18 years old and reside in the United States, where the Contest is legally permitted. Employees, officers, and representatives of Dunk Health and its affiliates, subsidiaries, advertising, promotion agencies, and their immediate family members or persons living in the same household are not eligible to participate in the Contest.
2. Contest Period: The Contest begins on July 15, 2023, and ends on September 30, 2023, at 11:59 PM Eastern Time (the "Contest Period").
3. How to Enter: To enter the Contest, follow these steps:
   • Leave a Review: During the Contest Period, participants must access the Dunk Health web application and leave a genuine review on Dunk Health's official Google Business page.
   • Refer Friends: For each referred person who successfully downloads the app, becomes a Dunk Health client by purchasing an insurance policy, the referring participant will earn one additional bonus entry into the Contest.
4. Prize Details: The total prize pool for the Contest is $500, to be distributed among three winners as follows:
   • First Place: $300
   • Second Place: $150
   • Third Place: $50

All winners will be selected randomly from all eligible entries received during the Contest Period. The odds of winning depend on the total number of eligible entries received.

1. Winner Notification: The winners will be notified via email within seven (7) days after the Contest ends. The potential winners will need to respond to the notification within seven (7) days from the date of notification to claim their prize. Failure to respond within the specified time will result in forfeiture, and an alternate winner will be selected.
2. Prize Distribution: Prize money will be distributed to the winners via the preferred payment method specified by Dunk Health. Winners may be required to provide additional information for prize distribution, such as a valid government-issued ID, for verification purposes.
3. General Conditions:
   • Participation in the Contest constitutes the participant's full and unconditional agreement to these official rules and decisions made by Dunk Health, which are final and binding.
   • Dunk Health reserves the right to disqualify any participant in violation of these official rules or involved in any fraudulent activities related to the Contest.
   • Participants agree to release, discharge, and hold harmless Dunk Health, its affiliates, subsidiaries, advertising and promotion agencies, and their respective officers, directors, employees, representatives, and agents from any claims, losses, or damages arising out of or relating to their participation in the Contest or acceptance, use, misuse, or possession of any prize received.
   • The Contest is void where prohibited or restricted by law.
4. Privacy: By participating in the Contest, participants agree to Dunk Health's Privacy Policy, available at https://dunk.health/privacy
5. Sponsorship: The Contest is solely sponsored by Dunk Health.
6. Governing Law: The Contest and these official rules are governed by the laws of the State of Florida, USA. Any disputes arising out of or relating to the Contest shall be subject to the exclusive jurisdiction of the courts in Florida.
